A Piece of Paper and a Call to Home

I was moving some books from one shelf to another (the ongoing saga of a bibliophile) when a small piece of paper fell out of one. I must have tucked this little memento inside a random book, but I do not recall having seen it before. It is handwritten and in German. I can translate some words, but many require further translation help.
